Upadacitinib Monotherapy for Treatment of Pyoderma Gangrenosum

Abstract

Introduction Pyoderma gangrenosum (PG) is an inflammatory disease characterized by sterile, neutrophilic infiltration of the dermis, resulting in painful lesions.1,2 It is difficult to diagnose and is typically a diagnosis of exclusion.2 Treatment usually involves immunosuppressants; however, relapses are common, and there is no established standard of care.2 Some studies indicate that up to 50% of patients have an associated systemic disease such as inflammatory bowel disease, rheumatoid arthritis, or hematologic malignancies.2 Janus kinase (JAK) inhibitors, such as upadacitinib, are a growing class of agents that have been used in the treatment of inflammatory conditions such as atopic dermatitis and psoriasis.3,4 Upregulation of the JAK/STAT pathway has been implicated in the development of PG, and limited case studies have reported success with the use of JAK inhibitors in managing these ulcers.5
