Can Statistical Data Replace Pathophysiologic Rationale?: A Belated Question for the Second Quarter of the 21st Century
Abstract
Dear Editor, I feel that it’s timely to bring an important question to your readers’ attention on a recent article published in your journal.1 Is sophisticated statistical data or mechanism-based clinical reasoning shaping our clinical judgment?. An easy answer would be they both are, but the truth on the ground tells us otherwise. In 1990s, oral inotrope digoxin and ACE inhibitor enalapril were jointly considered as first-line therapy in the treatment of heart failure (HF). In 1997, digoxin was declared optional, and soon a beta blocker carvedilol took its place.2,3 In the next 2 decades, seeking mechanistic justification for such important paradigm shifts became increasingly more scarce or was inadequately emphasized. Yet we must remember them and share with colleagues and our future physician workforce.
